Passing Member Form

The International Board seeks your assistance in ensuring that we are promptly informed in the unfortunate event of a member’s passing. In such cases, we kindly request that the Member Passing Form (Heaven One Membership Request) be completed.

As there is currently no official notification process at the Chapter level, we urge any member who becomes aware of another member’s passing to notify us without delay. Timely communication is essential for us to observe our funeral protocol and extend due respect and condolences to the bereaved family.

We deeply appreciate your cooperation and support in helping us honor our departed members with the dignity they deserve.
